Just an idea that I ran across in a story for another franchise and I figured I'd adapt it for Dr Who and post it here for anyone interested.
On the homeworld of one of the oldest races (Sontarans, Ruthans, etc) is a cell and in that cell sits a being, It say's nothing and never moves just sitting in the cell waiting. It is a great old one from the universe before this one. While others have used their power for personal gain or to try to go back it believes that all things are cyclical and waits for the end of the universe in the belief that when it ends another will be born again and again until its own universe is reborn. Then it senses a time travelling device (the party) and moves intending to use it to jump to the utter end of the universe so that it may cross to the next, an event the party is unlikely to survive or be able to return from.

I imagine while it has been waiting it has merely been counting the seconds/days/months/years to pass the time. It’s gone beyond recognisable digits by now.

I was picturing a cosmic scale where it sensed the time lords long ago but its spent a few million years thinking about what to do. It's not particularly rushed just the sort of vague Well I guess I can do this. If you've ever had a day where your just lounging at home and decide to do something like get lunch. Its not particularly urgent and if something else comes up you can grab a sandwich at home instead. It was waiting . . . oh what's that? . . . time travel? . . . well I guess that can knock a few eons off where's a ship they used to be all over . . . ah here's one welp time to get up. It didn't even notice the races attempts to kill it until they just built the jail around it just as it wont notice said jail when it leaves.

An entire race and culture dedicated to containing and guarding this monstrosity. Only for the monstrosity to suddenly just get up and walk off, not even giving them a second thought.
