Well the Whoniverse has so many options for duplicates. 1. Time travel: the same person earlier or later in their timeline 2. Disguise: someone with a shimmer or similar gadget 3. Robot duplicate: popular with the Daleks 4. Shapeshifter: like the unlamented Kamelion, Meglos, Frobisher et cetera 5. Clone: as done to Martha by the Sontarans and the Gangers 6. Weird Alien Tech: the Chameleons, Omega, Doctor 10½ 7. Natural resemblance: Ann Travers/Nyssa, Doctor 1/Abbot of Amboise
and more.

Your point seven (natural resemblance) is what I was getting at, where it's just a quirk of fate, a co-incidence in the infinity large, heavily populated, universe of Doctor Who, that two people happen to lookalike.

Or maybe it's not... The Whoniverse has had many forces manipulating events behind the scenes. Or it could be downb to <insert technobabble involving Quantum effects and Morphogenic Fields>.

Certainly happens in real life, and not even involving identical twins. There are people who make their living from their resemblance to famous people.
