I was rewatching this and it only just hit me Earth is still a level 5 planet yet all the passengers and crew look human. So obviously they aren't they're just an example of the doctor's lots of beings look human. So what do you think they were, for that matter why when humans are jsut a primitive level 5 planet do they have university courses on them?

In game terms, they have the Alien trait but nothing else about them to distinguish them from humans of Earth.
Doctor Who has always been happy to be vague about the prevalence of the human form on other planets. Are they from Earth colonies? Were humans of Earth brought back in time and transplanted to other planets? Does the human form just occur all over the place in the universe? (This is what I would assume.)

Aliens who are indistinguishable from humans and have no unusual traits don't need the Alien trait. The human-looking passengers on the Titanic are most likely statted as if they are in fact human.
Outside the TV continuity, the reason for the prevalence of the human form is down to the Time Lords, who meddled with the morphic field of the universe to ensure that most sentient races who evolved after them would be in their image.
