I don't have time to go wading through sourcebooks or discussion forums where this topic may have come up, but a question occurred to me about game styles. Given that it's pretty common for the Doctor to travel with just one companion or less commonly two, it seems to me that one option for a larger gaming group is to run a lot of one-off games in which the other players are "guest stars," e.g. playing the sorts of characters that would normally be friendly NPCs, or friends and allies in one particular story like Bishop Octavian in The Time of Angels and Flesh and Stone. In the following adventure the same players could take on different supporting roles or swap with whoever's playing the Doctor and companion(s). I think it could be an interesting alternative to the usual adventuring group arrangement.

I've heard similar suggestions in other games as well. Especially in games like D&D when you have a story line centered around one or two characters separate from the main party. It does allow a PC to get a chance to be the focus, as long as the game master is fair and gives equal time to all the players.
One way to make it work in a big group might be to have everyone in the group each have a Doctor they play, and you switch out every adventure.

I'd want the option to be there for everyone to have a go at being the Doctor as well.
